RESUMO

Renal artery stenosis of the kidney allograft associated with kinking is not a frequent finding. As a correctable cause of graft dysfunction, it is important to diagnose it as soon as possible to avoid further graft damage and improve graft and patient survival. As pulsed wave Doppler ultrasound mapping of the graft's renal arteries is essential to diagnose possible alterations, point of care ultrasound (POCUS) is a highly useful tool for early diagnosis. We present a case in which nephrologists performed this examination promptly allowing a timely diagnosis and treatment plan.

BACKGROUND: Within peritoneal dialysis (PD) complications, peritonitis remains a primary challenge for the long-term success of the technique. Proper technique training is essential, since it reduces peritonitis rates, but the adequacy of training has not been standardized. Furthermore, factors influencing training duration have not been well identified. METHODS: We retrospectively analyzed all consecutive training sessions of incident PD patients in our Unit from January 2001 to December 2018. RESULTS: Our analysis included 135 patients, 25.9% were diabetic and median Charlson index (CCI) was 4 (IQR 2-6). Above 13 sessions was chosen as the cut off between usual and prolonged training, as it was our cohort's 75th percentile: 23% (31) had an extended training duration as per our study definition and 77% (104) had a usual training duration. The number of training sessions required increased with age (Spearman Rho 0.404; p = 0.000001), diabetic status (p = 0.001), unemployment status (p = 0.046) and CCI (Spearman Rho 0.369; p = 0.00001). Neither gender, cohabitation status, scheduled PD start, education level nor referral origin, were significant factors impacting training duration. Requiring longer training (> 13 sessions) was a significant risk factor for higher peritonitis risk, but extended training was not related to a shorter technique survival. CONCLUSION: Number of PD training sessions depends on the patient's age and comorbidities, but is not related to social, educational or employment status. Prolonged training duration was a statistically significant predictor of higher peritonitis risk, but it was not related to shorter permanence in PD in our series. Identifying these patients since the training period would be useful to adapt training schedule as an early prevention strategy to minimize the risk of peritonitis and plan a preemptive retraining.

BACKGROUND: Thrice-weekly haemodialysis is the usual dose when starting renal replacement therapy; however, this schedule is no longer appropriate since it does not consider residual renal function. Several reports have suggested the potential benefit of beginning haemodialysis less frequently and incrementally increasing the dose as the residual renal function decreases. However, all the data published so far are from observational studies. Thus, this clinical trial avoids any potential selection bias and will assess the possible benefits that have been observed in observational studies. METHODS/DESIGN: This report describes the study protocol of a randomized prospective multi-centre open-label clinical trial to evaluate whether starting renal replacement therapy with twice-weekly haemodialysis sessions preserves residual renal function better than the standard thrice-weekly regimen. We also explore other clinical parameters, such as concentrations of uremic toxins, dialysis doses, control of anaemia, removal of medium-weight uremic toxins, nutritional status, quality of life, hospital admissions and mortality. Only incident haemodialysis patients who can maintain a urea clearance rate KrU ≥ 2.5 mL/min/1.73 m2 are eligible. Patient recruitment began on 1 January 2017 and will last for 2 years or until the required sample size has been recruited to ensure the established statistical power has been reached. The minimum follow-up period will be 1 year. Anuric patients with acute renal failure and patients who return to haemodialysis after a kidney transplant failure are excluded. It has been calculated that 44 patients should be recruited into each group to achieve a power of 80% in a two-sided comparison of means with a usual significance level of 0.05. A time-to-event analysis will estimate the probability of kidney function survival in both groups using the Kaplan-Meier method. Survival curves will be compared with log-rank tests. This survival analysis will be complemented with a proportional hazard model to estimate the hazard ratio of kidney function survival adjusted for any confounding factors. Analyses will be carried out in accordance with the intention-to-treat principle. DISCUSSION: The incremental initiation of dialysis may preserve residual renal function better than the conventional treatment, with similar or higher survival rates, as reported by observational studies. To our knowledge, this is the first clinical trial to evaluate whether initiating renal replacement therapy with twice-weekly haemodialysis sessions preserves residual renal function better than beginning with the standard thrice-weekly regimen. El acceso vascular para hemodiálisis es esencial para el enfermo renal tanto por su morbimortalidad asociada como por su repercusión en la calidad de vida. El proceso que va desde la creación y mantenimiento del acceso vascular hasta el tratamiento de sus complicaciones constituye un reto para la toma de decisiones debido a la complejidad de la patología existente y a la diversidad de especialidades involucradas. Con el fin de conseguir un abordaje consensuado, el Grupo Español Multidisciplinar del Acceso Vascular (GEMAV), que incluye expertos de las cinco sociedades científicas implicadas (nefrología [S.E.N.], cirugía vascular [SEACV], radiología vascular e intervencionista [SERAM-SERVEI], enfermedades infecciosas [SEIMC] y enfermería nefrológica [SEDEN]), con el soporte metodológico del Centro Cochrane Iberoamericano, ha realizado una actualización de la Guía del Acceso Vascular para Hemodiálisis publicada en 2005. Esta guía mantiene una estructura similar, revisando la evidencia sin renunciar a la vertiente docente, pero se aportan como novedades, por un lado, la metodología en su elaboración, siguiendo las directrices del sistema GRADE con el objetivo de traducir esta revisión sistemática de la evidencia en recomendaciones que faciliten la toma de decisiones en la práctica clínica habitual y, por otro, el establecimiento de indicadores de calidad que permitan monitorizar la calidad asistencial.


Vascular access for haemodialysis is key in renal patients both due to its associated morbidity and mortality and due to its impact on quality of life. The process, from the creation and maintenance of vascular access to the treatment of its complications, represents a challenge when it comes to decision-making, due to the complexity of the existing disease and the diversity of the specialities involved. With a view to finding a common approach, the Spanish Multidisciplinary Group on Vascular Access (GEMAV), which includes experts from the five scientific societies involved (nephrology [S.E.N.], vascular surgery [SEACV], vascular and interventional radiology [SERAM-SERVEI], infectious diseases [SEIMC] and nephrology nursing [SEDEN]), along with the methodological support.

Introducción: El objetivo de este trabajo es destacar la frecuencia e importancia de las perdidas óseas glenoideas que no fueron consideradas por mucho tiempo y las posibilidades terapéuticas teniendo en cuenta dicha lesión. Presentar nuestra experiencia en el tratamiento estandarizado de las luxaciones recurrentes anteriores del hombro, vírgenes de cirugía previa, enfatizando en las pérdidas óseas antero inferiores del reborde glenoideo. Objetivo: Determinar la importancia de las pérdidas óseas de la glenoides en inestabilidades recurrentes de hombro. Material y Métodos: Se estudiaron 22 pacientes sin tratamiento quirúrgico previo por la patología descripta, con edades entre 18 y 45 años, con un claro predominio del sexo masculino (proporción 8 a 2). Se operaron con la técnica abierta modificada de Bristow Latarjet. La base de nuestro enfoque es la correcta selección de los pacientes, la técnica aplicada y el programa de rehabilitación precoz. La serie la consideramos de homogeneidad aceptable entre sus integrantes, así como los procedimientos quirúrgicos y la evaluación efectuada. Resultados: Se realizó una evaluación retrospectiva objetiva y subjetiva de los resultados a los dos años en una cohorte de 22 pacientes. Se valoraron de acuerdo con exámenes físico, imagenológico y de resultados, por cuestionarios completados por médicos y pacientes con el Score de Constant y de la ASES. Las cifras obtenidas fueron: 86% presentó hombros estables, indoloros o con poco dolor con una mínima pérdida de la rotación externa considerada entre los 6 y 8°, con limitaciones menores para las actividades por encima de la cabeza. No observamos re luxaciones o problemas con la consolidación de la coracoides ni tampoco artrosis temprana. Los consideramos resultados excelentes o buenos. Los mismos encuentran su principal objeción en la falta de verificaciones a largo plazo, que es cuando puede observarse la aparición de artrosis secundaria...


Background: Treatment of recurrent anterior shoulder instability isin continuous development. This disabling condition requires allthe possible knowledge, skills and broad experience to be solved. The purpose of this study was emphasized the importance of the glenoid bone loss and to verify th e hypothesis that the open coracoid transfer as described by Bristow Latarjet is a highly successful for treating recurrent glenohumeral instability associated with glenoid bone losses.Objectives: To determine glenoid bone losses importance inshoulder recurrent instability. Material and Methods:We selected a cohort of 22 patients, ages between 18 and 45 y.o. with predominance of males over females (rate 8 to 2), with recurrent anterior glenohumeral instability. All of them were treated with open Bristow Latarjet technique. INTRODUCTION: The aim of this work was to study the accuracy of the CKD-EPI equation to estimate the glomerular filtrate in patients with advanced chronic renal failure. OBJECTIVE: We compared the estimations of Modification of Diet in Renal Disease (MDRD), Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) and Cockcroft-Gault (CG) equations to a glomerular filtration rate measured as the arithmetic mean of the urea and creatinine clearances (ClUrCr). MATERIAL AND METHODS: The study was made in 89 nondialyzed patients with chronic renal disease in stage 4 or 5. Serum creatinine values were recalibrated to standardized creatinine measurements. In each patient, the difference between each estimating equation and the measured glomerular filtration rate was calculated. The absolute difference expressed as a percentage of the measured glomerular filtration rate indicates the intermethod variability. RESULTS: Overall, the glomerular filtration rate measured as the ClUrCr was 14.5 ± 5.5 ml/min/1.73 m(2); and the results of the estimating equations were: MDRD 14.3 ± 5.5 (p = NS); CKD-EPI 13.6 ± 5.4 (p <0.01) and CG 16.8 ± 6.5 ml/min/1.73 m(2) (p <0.001). The variability of the estimating equations was 16 ± 12.2%, 16.7 ± 12,1% and 22 ± 15.6% (p <0.05), for MDRD, CKD-EPI and CG. The percentage of estimates within 30% above or below the measured glomerular filtration rate was 85% for MDRD, 88% for CKD-EPI and 70% for CG. The CG variability, but not MDRD variability or CKD-EPI variability, was influenced by gender (19.3 ± 15.1% in males vs 27.3 ± 15.5% in females, p <0.05) and showed a negative correlation with the glomerular filtration rate (r = -0.23, p <0.05) and the age (r = -0.24, p <0.05) and positive correlation with the body mass index (r = 0.37, p <0.001). In patients with chronic renal disease in stage 5, the variability of the different estimating equations was similar. CONCLUSIONS: We conclude that in our population with advanced chronic renal failure, the CKD-EPI equation is as accuracy as the MDRD equation. With standardized creatinine the CG equation has a lower accuracy and its utilization may be reconsiderated.

Half of patients starting chronic hemodialysis used a transient vascular catheter as a vascular access (unplanned initiation). An objective of the Quality Management Group of the Spanish Society of Nephrology is to achieve that 80% of the patients starting hemodialysis do it with an arteriovenous fistula. We want to review the causes of non-planned hemodialysis nowadays. In 2010, 43 patients had started chronic hemodialysis in the Hospital Ramón y Cajal in Madrid (Spain). Mean age was 61 years, 79% were men, the most frequent cause of chronic renal disease was the diabetes (23%) and Charlson Comorbidity Index was 6.3 ± 2.6. The unplanned hemodialysis occurred in 20 patients (47%), without any differences with the 23 patients who began planned hemodialysis, in none of the clinical or demographic parameters analyzed. The main cause of unplanned hemodialysis was the acute exacerbation of chronic kidney disease stage 3 or 4, previously stable, secondary to an unforeseeable intercurrent process (8 patients, 40% of the cases). One patient began after a non-recovery acute renal failure and in other 6 patients, the reason of unplanned hemodialysis initiation was not attributable to the operation Health System (in 3 cases unknown kidney chronic disease and in the other 3 cases it was patient´s responsibility). Only in 5 cases (25%), the cause could be corrigible. Most causes of unplanned hemodialysis does not come from the healthcare organization and therefore not easy to resolve it. Consequently, the objective of the Quality Group will be difficult to be achieved.

Introducción: La ecuación clásica Modification of Diet in Renal Disease (MDRD) subestima el filtrado glomerular en los valores más altos. Para aumentar su concordancia en los estadios 1 y 2 de la enfermedad renal crónica se ha establecido una nueva fórmula: la ecuación Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI). Objetivo: El objetivo del presente estudio es comprobar si la nueva ecuación es mejor que la MDRD en la estimación del filtrado glomerular en la insuficiencia renal crónica avanzada, en una población como la nuestra que tiene unas características antropométricas distintas de las de la población norteamericana. Material y métodos: En 89 pacientes con enfermedad renal crónica en estadios 4 y 5, hemos estudiado el grado de concordancia entre el filtrado glomerular medido como la media de los aclaramientos de urea y creatinina (AclUrCr) y el estimado por las ecuaciones MDRD, CKD-EPI y Cockcroft-Gault (CG). La concentración de creatinina corresponde a creatinina estandarizada. Introducción: Los pacientes trasplantados renales con disfunción crónica del injerto que precisan reinicio de diálisis constituyen un grupo cada día más prevalente, con características especiales que los diferencia de la población general con enfermedad renal crónica (ERC). El objetivo del presente estudio fue analizar y comparar la situación clínica al inicio de diálisis y su evolución en el primer año endos grupos de enfermos, trasplantados y no trasplantados, de acuerdo con los criterios fijados en las guías K/DOQI. Asimismo, se estudia si la modalidad de terapia renal sustitutiva (TRS) escogida a su retorno a diálisis pudiera condicionar una mejoría clínica de los pacientes trasplantados. Material y métodos: Estudio retrospectivo y observacional en 106 enfermos con ERC estadio 5 seguidos en el Hospital Ramón y Cajal. Dichos enfermos fueron divididos en dos grupos. El primero estaba formado por 50 enfermos con ERC de riñones nativos que comenzaron tratamiento con diálisis entre los años 2000 y 2009. El segundo grupo estaba constituido por 56 enfermos trasplantados con disfunción del injerto que precisaron retorno a diálisis entre los años 1997 y 2009. Se recogieron parámetros de anemia, función renal, metabolismo calcio-fósforo, factores de riesgo cardiovascular y estado nutricional en el momento de comenzar tratamiento con diálisis y un año después. These patients have special characteristics that set them apart from the ESKD general population. The aim of this study was to analyse the clinical condition and evolution of patients entering dialysis with a failed kidney graft at the time of restarting dialysis and over a year of therapy according to the K/DOQI guidelines, and to compare them with incidental patients with end-stage kidney disease. We also investigated whether the modality of kidney replacement therapy may determine the clinical improvement of transplant patients. MATERIAL AND METHOD: This is a retrospective observational study of 106 patients with ESKD followed up in the Ramon y Cajal Hospital. They were classified in two groups. Group one was made up of 50 failed native kidney patients who started dialysis between 2000 and 2009. Group two was comprised of 56 transplant patients with graft dysfunction who returned to dialysis between 1997 and 2009. We studied parameters of kidney function, anaemia, calcium-phosphorus metabolism, cardiovascular risk factors and nutritional status at the time both groups started on dialysis and one year later. RESULTS: Both groups had a similar clinical status at the time they started on dialysis in most of the parameters analysed with the exception of anaemia. This was more severe in transplant patients, despite the fact that transplant patients received a higher dose of erythropoietin than non-transplant patients. One year later the main difference between both groups was the residual kidney function rate, higher in non-transplant patients. There were no significant differences in the parameters analysed in patients with a failed graft according to the modality of kidney replacement therapy. CONCLUSION: Failed transplant patients start dialysis with more severe anaemia than patients entering dialysis for the first time. Twelve months later both groups present a similar clinical condition with the exception of residual kidney function, higher in failed native kidney patients. The method of dialysis treatment after kidney transplant failure did not have a bearing on the clinical improvement of our patients.

Introducción: La anorexia es un trastorno frecuente en el enfermo tratado con hemodiálisis periódica, y factor contribuyente de la malnutrición. El objetivo del presente trabajo es comprobar la eficacia del acetato de megestrol, un estimulador del apetito utilizado en enfermos con cáncer, como tratamiento de la anorexia del enfermo sometido a diálisis. Material y métodos: En el año 2009, 16 enfermos de nuestra unidad de hemodiálisis, tres de ellos con diabetes mellitus, fueron tratados con acetato de megestrol (160 mg/día en dosis única), por anorexia definida según una escala Likert de apetito. La pauta y la dosis de diálisis no fueron modificadas durante el estudio. Resultados: Al tercer mes de tratamiento se objetivó, en el grupo total, un aumento del peso seco (60,8 frente a 58,9 kg; p <0,01), de la concentración de albúmina (4,02 frente a 3,8 g/dl; p <0,05), de la concentración de creatinina (9,73 frente a 8,26 mg/dl; p <0,01) y de la tasa de catabolismo proteico (1,24 frente a 0,97 g/kg/día; p <0,001). No hemos constatado variaciones significativas en la concentración de hemoglobina, dosis de eritropoyetina y concentración de lípidos. En un enfermo con diabetes mellitus hubo que aumentar la dosis de insulina y en otros 2 enfermos se detectó una hiperglucemia leve. El acetato de megestrol no suprimió la secreción de hormonas sexuales hipofisarias, pero en 3 de 10 enfermos estudiados se constató una inhibición de la secreción de corticotropina. La respuesta no fue homogénea: un enfermo no respondió y disminuyó su peso seco, en cinco el incremento de peso fue discreto (inferior a 1 kg) y en los 10 restantes la respuesta fue buena, con un incremento de peso seco que osciló entre 1,5 y 5,5 kg. Conclusiones: El acetato de megestrol puede mejorar el apetito y los parámetros nutricionales en enfermos tratados con hemodiálisis periódica que refieran anorexia. El acetato de megestrol puede inducir hiperglucemia e inhibir la secreción de corticotropina en algunos pacientes. Estos efectos secundarios deben ser valorados cuando se administre este tratamiento (AU)

End-stage renal diseases (ESRD) are becoming more frequent in HIV-infected patients. In Europe there is little information about HIV-infected patients on dialysis. A cross-sectional multicenter survey in 328 Spanish dialysis units was conducted in 2006. Information from 14,876 patients in dialysis was obtained (81.6% of the Spanish dialysis population). Eighty-one were HIV infected (0.54%; 95% CI, 0.43-0.67), 60 were on hemodialysis, and 21 were on peritoneal dialysis. The mean (range) age was 45 (28-73) years. Seventy-two percent were men and 33% were former drug users. The mean (range) time of HIV infection was 11 (1-27) years and time on dialysis was 4.6 (0.4-25) years. ESRD was due to glomerulonephritis (36%) and diabetes (15%). HIV-associated nephropathy was not reported. Eighty-five percent were on HAART, 76.5% had a CD4 T cell count above 200 cells, and 73% had undetectable viral load. Thirty-nine percent of patients met criteria for inclusion on the renal transplant (RT) waiting list but only 12% were included. Sixty-one percent had HCV coinfection. HCV-coinfected patients had a longer history of HIV, more previous AIDS events, parenteral transmission as the most common risk factor for acquiring HIV infection, and less access to the RT waiting list (p < 0.05). The prevalence of HIV infection in Spanish dialysis units in 2006 was 0.54% HCV coinfection was very frequent (61%) and the percentage of patients included on the Spanish RT waiting list was low (12%).
